Which treatments do you have forms for?

There is one form per treatment, each with a client file, the medical history and a consent page: BB Glow, Facial & Skincare, PMU, Hyaluron Pen, Mesotherapy, Plasma Pen, IPL Hair Removal and Laser Hair Removal. General forms work alongside any treatment: COVID-19, GDPR Consent and Contraindicated Meds.

What exactly do I get?

For each form you buy, one ZIP file with a fillable PDF in A4 format, an editable PowerPoint file (.pptx) and a short guide to customising them. They are digital files — nothing is posted to you.

How much does a form cost?

Each form costs €12,00 (regular price €15,00). Prices are in euro, the total shown at checkout is the amount you pay, and there are no delivery charges.

When can I download my forms?

As soon as the payment is confirmed. The download links appear on the order confirmation page, in your order email and, if you have an account, in My account → Downloads. Each download link works up to 5 times within 14 days of purchase (the IPL Hair Removal Form: once within 5 days of purchase).

Do I need an account to buy?

No. You can buy as a guest — the download links come on the confirmation page and in your order email. With an account you can also see your orders and download your forms again from My account.

Which apps do I need?

For the PDF, a reader that supports form fields — for example the free Adobe Acrobat Reader, or Preview on a Mac. For the .pptx file, Microsoft PowerPoint. Other presentation apps can open it too, but the layout may shift.

Can I put my salon’s logo on the forms?

Yes, in the PowerPoint file: replace the logo placeholder with your own, change the colours from the theme and edit the text. Then export it to PDF.

Can I print the forms?

Yes. They are laid out on A4 — print them at 100% (actual size) and fill them in by hand.

Which language are the forms in?

English. In the PowerPoint file you can change the wording yourself.

How can I pay?

By card through Stripe, or with PayPal. Card details go straight to the payment provider — we never see or store them.

Can I get an invoice for my salon?

Yes. Write to office@beauty-box.shop with your order number and your company details to get an invoice made out to your business. See business customers in our terms.

Can I return a form?

Digital files are delivered straight away, so at checkout we ask you to agree that the download can start immediately; with that agreement the 14-day right of withdrawal no longer applies (Art. 16(m) of Directive 2011/83/EU). Your rights for faulty digital content still apply — if a file does not open or does not match its description, write to office@beauty-box.shop. Details: Right of withdrawal.

Do the forms cover the rules in my country?

They are templates. Requirements for client records and consent differ from country to country, so check with your local authority or your insurer what your salon has to record, and adjust the PowerPoint file if needed.

Can I share the forms with another salon?

The forms are for use in your own salon: print and edit them as often as you need, but do not resell or pass on the files. See the licence in our terms.
